Handover experience taught me the risks that lie in listening to gossip
At the start of a morning shift, I was present as a night nurse handed over details of a patient, Mr Smith, to the ward’s day staff.
An effective handover should include all the relevant patient information such as diagnosis, past medical history, investigations and resuscitation status. it should also include any issues relevant to nursing care, such as continence and hygiene needs, nutrition and hydration, and communication issues.
